I have applied surcharges under these circumstances:

1. Tight deadlines;
2. Work done during the weekend;
3. Certification (lawyer fees); I don’t accept the project if it has to go through a notary (it’s a time waster).

I don't accept projects with heavy formatting. Regarding splitting a project, I only accept it with some trusted colleagues and usually I don’t charge extra.

I usually don't work on weekends, or if I do, it's because I decide that I want to work for whatever reason. However, I would also accept work over the weekend for a surcharge.
As for tight deadlines, I actually thrive on them as long as they are physically doable, so I wouldn't apply any surcharge for that.
When it comes to formatting, that's one of the biggest upsides of working with CAT tools and translation packages: formatting becomes a non-issue because I do not even get to see

Anything extra takes more of my time and will therefore cost extra.

By the way, even the big agencies are happy to pay extra for extra work. You just have to ask them.

I don't mind working weekends especially if I haven't worked in the week. A lot of my friends are either retired or work at the weekend so we can meet up during the week, the notion of weekends doesn't really apply to me any more.
However, clients don't know this. If they ask me to do a job that would require weekend work, or working late into the evening or having to get up early in the morning to get a job done, I will apply an emergency rate. It doesn't happen very often at all though

I’d apply them for annoying, disorganized clients hoping that will make them go away.

Exactly, especially if they already know and trust you or there isn't a lot of time to find somebody else. I've heard occasional "I am very sorry, but our budget is really limited for this", but even then they usually try to meet somewhere half way. A lot more often I have asked a higher rate hoping to get rid of something I didn't want to do and they've accepted it immediately.

I am so much used to heavy formatting that it goes flawless as a piece of cake. I have attached a sample of a quite heavy formatting. I had to use tabloid paper to do it.
